Acne and Scar Treatment

Diagnosis: 20 year old with micro-nodular acne and scarring
Treatment: Initially treated with isotretinoin and radio frequency treatment. Subsequent scar treatment with combination Erbium:YAG and CO2 laser for general scarring and TCA CROSS procedure for ice-pick scars.

Case 15 - Erbium and CO2 Fractional combination laser

Diagnosis: This patient previously had 3 Fraxel treatments by a ‘Cosmetic GP’ with little improvement. There is no consensus on this form of acne scars, and I would class this as mixed acne scars.
Treatment: I performed Erbium laser to a very deep level- over 900 microns full ablation. This equates to over 9 times the depth of the upper levels of the skin. I then used Fractional CO2 and performed 5 passes. Healing time was rather surprising- 8 days till the skin healed. We were both very happy with the outcome.

Case 14 - TCA and Laser - perfect combination

Diagnosis: This patient had ice-pick & box car scars predominantly isolated to her cheeks.
Treatment: Once again TCA was used to raise the deeper ice pick and box-car scars to one level. This was followed by one session of erbium and factional CO2 laser.

Diagnosis: This patient had ice pick, box car and mixed scarring on the cheeks.
Treatment: Once again TCA was used to raise the deeper ice pick and box-car scars to one level. This was followed by one session of erbium and factional CO2 laser.

Case 12 - TCA and Laser - perfect combination

Diagnosis: This patient had deep box car and ice pick scarring on the cheeks.
Treatment: I elected to use TCA CROSS (2 treatments) to raise deep scars, followed by one session of erbium and my favourite laser- the CO2 CORE in fractional mode. She still has 3 tiny scars remaining, and ideally I would have liked to perform one more TCA, but she was happy as is.

Case 10 - Atrophic and rolling scars

Diagnosis: Difficult case. Mixed scars, atrophic scars, rolling and anchored scars.
Treatment: Fractional laser combined with subscision and dermal fillers. This program took 4 months to complete. Further improvements can be made with either superficial filler, and or more laser fractional treatments.

Case 7 - Mixed acne scars

Diagnosis: Mixed acne scars. This patient had FRACTIONAL RESURFACING before seeing me- hence the grid like patterns on top on acne scarring. I had to remove the grids and the acne scars.
Treatment: Full laser resurfacing (ERBIUM), no Fractional component was used due to pre-existing grids. Downtime of 9 days. Followed up with 3 VBeam vasulcar laser treatments to aid recovery and decrease redness. Marked improvement in pore size, grids, texture and scars.

Case 6 - Moderate to severe mixed acne scars

Diagnosis: Moderate to severe mixed acne scars-element of hypertrophic and rolling scars.
Treatment: Full laser resurfacing (ERBIUM) coupled with Fractional Laser all in one treatment. V-Beam vascular laser and one RF treatment were conducted following full laser resurfacing. Great improvement in acne scarring. Picture 2 shows recovery 6 days follow laser resurfacing. Downtime of 8 days.

Case 5 - Scarring in Asian skin type

Diagnosis: Scarring in Asian skin type. Note - mixed scars, ice pick and box car scars.
Treatment: TCA Cross to Ice Pick scars and pores, followed by full erbium laser resurfacing + Fractional laser during the same procedure. Downtime of 5 days following TCA Cross, and 8 days following laser.

Case 4 - Severe mixed acne scars

Diagnosis: Mixed acne scars - severe. Atrophic, rolling, tethered and anchored acne scars in male.
Treatment: Low dose isotretinoin coupled with 5 fractional RadioFrequency and fractional laser treatments . Downtime of 2- 3 days following fractional treatments. Marked improvement in acne scarring.

Case 3 - Moderate to severe mixed acne scars

Diagnosis: Moderate to severe mixed acne scars cheeks of 24 year-old female.
Treatment: Full laser resurfacing coupled with Fractional Laser + Subscision, all in one treatment. 4 treatments of V Beam vascular laser were conducted to reduce redness following full laser resurfacing. Excellent improvement in acne scars. Picture 2 shows recovery 24 hours follow laser resurfacing. Downtime of 8 days.

Case 2 - Severe cystic acne

Diagnosis: Severe cystic acne, treated with Isotretinoin and anti-inflammatories for several months. Remaining severe scarring- hypertrophic coupled with mixed scars.
Treatment: Full laser resurfacing with Erbium laser, followed by VBeam. 2 treatments. I used 3 RadioFrequency treatments to finish the program. Program period of 6 months. Maximal downtime of 9 days.

Treatment: Full laser resurfacing coupled with Fractional Laser. Followed by V Beam laser to reduce recovery time. 3 treatments of RadioFrequency. Program period 4.5 months. Downtime of 7 days following first procedure.
